Continuing Care Services at Piedmont Crossing

With its mission-driven focus, Piedmont Crossing offers compassionate health care services that include assisted living, healthcare, memory care, and rehabilitation. Piedmont Crossing offers compassionate care to meet the physical and safety needs of each person. For some residents, that care may be delivered in the comfort of their home through our home care service; other residents may need additional assistance.

The benefit of Piedmont Crossing’s lifestyle is access to different levels of care when they’re needed. This means that residents may utilize short-stay or respite services while recovering from an illness or surgery. Or residents who need added care may move to a longer-term setting on campus for assisted living or memory care. Should a couple have differing care needs, both can live on the same campus and be within a short walking distance of each other. The benefit of the Piedmont Crossing lifestyle is individual access to care at the right time, in the place the resident desires, and with proximity to friends and family.

Assisted Living

Assisted Living at Piedmont Crossing offers a supportive lifestyle for residents requiring assistance with daily activities such as bathing, getting to and from meals, dressing, and taking medications.

Our assisted living area allows residents who need some support with activities of daily living the convenience of these services in a private room with their own furnishings and 24-hour staffing. Residents pay a monthly fee that includes three meals each day, housekeeping, personal assistance, and social and recreational activities.

Skilled Nursing

With its cozy setting and home-like comforts, Piedmont Crossing Health Center offers nursing care on a temporary or long-term basis.

Our Medicare- and Medicaid-certified nursing center provides a full array of services to meet the acuity needs of each individual. Our professional staff provides personal and medical support to enhance our residents’ quality of life – physically, mentally, and spiritually.

Long-term Care

Piedmont Crossing Health Center also serves people who need longer-term health services. Its cozy setting and comfortable amenities welcome folks, and our team focuses on care plans that prioritize the best interests of the resident. At this level of living, couples with different needs can take advantage of differing levels of care and remain on the Piedmont Crossing campus together.

 

Home Health

Piedmont Crossing at Home provides in-home assistance with health matters and is licensed to serve Davidson, Rowan, Davie, Forsyth, Guilford, Randolph, and Montgomery counties. The service also helps with a variety of other needs and offers assistance with everyday tasks that would be difficult or unsafe when performed by the resident alone. Our team will assess each situation and create a plan to best meet the resident’s needs. Our goal is to inspire each individual to embrace life, reach their highest level of functionality, and live with meaning and satisfaction.

Memory Care

Piedmont Crossing offers a dynamic, innovative approach to caring for people with Alzheimer’s or related dementias. In addition to daily care, our passionate and experienced staff help residents participate in a variety of activities that are emotionally, mentally, spiritually, and physically engaging.

A daily rate covers a resident’s meals, housekeeping, 24-hour nursing care and supervision, and activities designed for people with dementia and memory loss.

Physical Rehabilitation & Therapy Services

Our goal is your goal: getting you back on your feet, enjoying the things you love. Piedmont Crossing’s rehabilitation care offers personalized treatment on an in-patient and out-patient basis.

Short-term rehab services offer comprehensive care for people recovering from surgery, including those who have had joint replacements or experienced serious illness or injury, fractures, stroke, and other medical conditions. A caring staff trained in physical, occupational, and speech therapy works with each person to help them reach their highest potential.

Pre-Registering for Services

Piedmont Crossing makes it simple for anyone to pre-register for short-term rehab or respite services at the Health Care Center. Simply complete this form and a member of the Transitional Services Team will follow up with the next steps.

Pre-registering for services allows a patient to choose a short-term rehab provider prior to surgery or in-patient acute admission. Patients are able to focus on recovery, knowing the transition to short-term rehab has already been determined.
